Just a quick reminder: all parents are required to show personal ID and proof of insurance at every visit, this is an FTC “Red-Flag Rule” designed to prevent identity theft and insurance fraud. Also, we ask that everyone completes/updates their contact information at least every 2 years. These “grey forms” are the only way we have of contacting you with lab or radiology results and if not filled out can lead to insurance companies denying your claims.
